The Health and Disability Commission's found the nurse in breach of the consumer rights' code.
Two brothers - similar in appearance and age - were inmates at the same prison, and scheduled at the health unit on the same day.
The man meant to get a hepatitis vaccination, was injected with an anti-psychotic medication intended for his brother.
The nurse failed to identify she had the wrong brother - even after a cross-checking process and having the man state his full name.
Deputy Health and Disability Commissioner Kevin Hall says it could have been prevented had the nurse told the man what she was administering.
