Mechanical faults caused a jet boat to crash into rock face in a river near Queenstown last year

The Discovery 2 - operated by Skippers Canyon Jet - was traversing a narrow section of the Shotover River when it crashed.
One passenger was thrown overboard, another's leg was broken - and others were left with minor cuts and bruises.
The Transport Accident Investigation Commission revealed the steering and propulsion system failed - when improperly tightened stud-bolts broke.
TAIC say drivers need to ensure they follow proper maintenance instructions.
