The man was hospitalised with severe abdominal pain and treated for gastritis but was discharged that evening.
On his return after worse pain overnight, he was diagnosed with reduced blood flow to his intestine and a blocked main artery.
He died two days later.
Health and Disability Commissioner Anthony Hill says deficiencies included delays in seeing a doctor.
He recommended an audit of ED wait times and documentation training for junior doctors.
The DHB has apologised to the man's family.
