A shoddy safety manual is being blamed for a death on a logging ship in Gisborne last year

Carrier ship Coresky OL had just finished loading logs at Eastland Port - when a cable snapped and fatally struck a crew member.
A Transport Accident Investigation Commission report released today reveals the cable was under too much tension - and ship's cargo securing manual had no safety guidance.
This left crew members unaware of the hazards involving wires and cables.
