A host of recommendations for Corrections and Clendon Pharmacy following the failed management of a prisoner's medication

The man was prescribed medication to reduce the risk of heart attack and stroke after he'd been hospitalised following a stroke.
What followed was a series of errors in stopping the medication and multiple hospital admissions - including after suffering a heat attack.
Deputy Health and Disability Commissioner Kevin Allan recommends both the pharmacy and Corrections write an apology - which they've done.
Other recommendations include arranging an independent review of the level of GP cover at Auckland Prison, and undertaking a random audit of pharmacy dispensing to the prison health centre.
The case has been referred to see if any legal proceedings should be taken against Corrections.
