Deputy Health and Disability Commissioner Rose Wall's report found the home's owner breached the Code of Health and Disability Services Consumers' Rights.
The woman had numerous health conditions and suffered a stroke.
She became delirious, dehydrated and feverish and developed a sore on her leg.
Staff didn't regularly monitor her condition before she was transferred to hospital - where she died of blood poisoning from cellulitis.
Wal's recommendations include developing better policies to support staff decision-making.
