The Health and Disability Commission's found a District Health Board and a urologist breached Health and Disability Services Rights in caring for a man in his 50s.
It's found crucial GP information wasn't made available to the urologist - and his symptoms weren't thoroughly investigated.
A year later he returned as a private patient but it took an emergency department visit and a new urologist, before cancer was diagnosed.
The Commissioner's recommending the parties apologise to the man, prepare a case study for staff training, and review processes.
