Tyler Nii died in January 2018 after a series of failures and malfunctions resulted in him and his tandem skydiving master crash landing into Lake Wakatipu.
It was recommended the Ministry of Transport work with the Civil Aviation Authority and parachuting umbrella bodies to mitigate risks of unintended water landings.
However, in March, British national Sarah-Jane Bayram died in an unintended water landing off Muriwai Beach after colliding with another parachutist.
Tyler Nii's older brother Kevin says it's upsetting to learn of another death and believes the CAA failed to act on one of the key safety recommendations resulting from his brother's accident.
