The German sixth seed won the first two sets 7-5 6-3 before losing the next two in a tiebreak and the fifth 6-3.
Zverev says he had a fever after beating Carlos Alcaraz and started to lose energy at the end of the second set.
Medvedev will meet Italian Jannik Sinner in tomorrow night's final.
