Nineteen-year-old Fyfa Dawson died in a crash during the construction of the Christchurch motorway at Halswell in October 2019.
Coroner Ruth Thomas found the temporary traffic management plan was vehicle-centric, forcing cyclists to use the pedestrian pathway - a blind spot for truck drivers.
She says there was also no one stationed at the work site gate and warning sensors weren't used.
Thomas recommends NZTA provide heavy vehicle users a free lens to help them see in their passenger side blind spot.
