    A family is calling for better information sharing between Government agencies


    Rob Fitzpatrick's death is one of three involving double bunking at Mt Eden Prison, in just over 12 months.

    He was allegedly beaten to death by a man who was accused of attacking someone near a Mangere bus stop.

    Corrections Custodial Services Commissioner Leigh Marsh says risk assessments are done for all prisoners - but that is not the answer to the issue.

    He says it is more about the processes after they are admitted - because situations are constantly changing.
