    A pharmacist failed to consult a man's GP three times before he suffered a fatal brain bleed


    The man, who was on blood thinners - started getting worsening headaches, describing them as extreme, with swishing noises in his ears.

    Open Justice reports he collapsed in September 2021 and his wife called an ambulance - but he couldn't be saved.

    The Deputy Health and Disability Commissioner has criticised a pharmacist for failing to speak to the man's GP when routine blood tests indicated something might be amiss.

    Among Carolyn Cooper's recommendations is an apology from the pharmacy, and changes for their software provider.
