A report by Health and Disability Commissioner Anthony Hill says the child, who was with his mother, was tired, restless, wriggling and became upset.
After the child kicked the GP, the GP inflicted contact on the child's face using all or part of her hand.
It was described as a 'slap' by the mother and a 'tap' by the GP.
Hill says the incident appears to be out of character for the GP and he did not consider it be reflective of her competence or likely to occur again.
The GP has undergone training and apologised to the child's mother.
