    Proof a tiny fault can cause disaster


    The Transport Accident Investigation Commission has concluded its inquiry into the crash of a jet boat carrying 12 passengers in Queenstown last year.

    The boat's engine stopped and the driver lost control on the Shotover River, colliding with a low tree branch.

    The driver and one passenger were injured - and flown to hospital.

    The TAIC found a control system fuse had failed.

    Maritime New Zealand is being advised to work with commercial operators to identify single points of failure that would result in a total loss of control.
